Scholarship Policy
Medicaid does not generally cover diabetes education. For individuals who do not have insurance, or those who have Medicaid, and who meet the guidelines included in the policy, there may be an opportunity for a scholarship to cover the cost of your diabetes education. Read this policy document to determine if you qualify.

Scholarship Application
If you qualify based upon the criteria outlined in the Scholarship Policy, you may be eligible for scholarship funds to cover the cost of your diabetes education. Complete this form to apply.


Supplemental Nutrition Assistance Program

If you're having trouble putting healthy food on the table, check out SNAP--the federal Supplemental Nutrition Assistance Program (formerly known as the Food Stamp Program) which assists with the cost of groceries.

  • What is the benefit?
    This program helps low-income individuals and families buy the food they need for good health. You get a special debit card that can be used like an ATM card at most grocery stores, certain senior centers, and non-profit meal delivery services (such as Meals on Wheels). The amount you get depends on your income, family size, and where you live.
  • Am I eligible?
    You may be eligible for this program if you have limited income and resources (not including your home, car, and certain retirement accounts such as: IRA, 401K, and Keogh). Citizens and certain legal immigrants who meet the eligibility guidelines, may be eligible for this program. Eligible non-citizens may include: immigrants who have legally lived in the United States for at least 5 years or more; immigrants currently receiving disability-related assistance or benefits, regardless of entry date; or any immigrant child, regardless of entry date.   • How do I apply?
